New Pay Grade for 2024!  

Hourly Range: $23.50 USD to $34.05 USD

Summary of Position: 

The Home Health Aide provides assistance to Home Health and Hospice patients with all aspects of personal hygiene, activities of daily living, nutrition, exercises and reports any significant changes to the Case Manager. This position utilizes established policies and procedures, ensuring safe and effective care. The Aide provides care as part of the interdisciplinary Home Health and Hospice team. Services are provided based on the age of the population served; Adults at least 18 years old through geriatric (65 years or older).

Qualifications

Education:

• Completion of Accredited Nursing Assistant and Home Health Aide courses required

Experience:

● One year experience as a Home Health Aide /Nursing Assistant preferred 

● Home Health and Hospice experience preferred

Knowledge/Skills/Abilities:

● Proficient computer skills are required to document, communicate and enter information into the electronic medical records system.

● Requires critical thinking skills, decisive judgment and ability to work with minimal supervision.

● Must be able to work in a stressful environment and take appropriate action.

● Excellent oral and written communication skills.

● Excellent interpersonal skills.

● Ability to collaborate with various departments in order to provide consistently exceptional patient care. 

● Attention to detail, meticulous development and maintenance of regulatory paperwork.

Certifications/Licensure:

● Current California and Nevada CNA/Home Health Aide certification

● Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date 

● Valid California or Nevada Driver's license (depending on state of employee's residence) or ability to obtain prior to start date 

● Proof of current vehicle insurance/registration

Essential Functions

1. Provides consistently exceptional care at all times.

2. Demonstrates the skill necessary to meet the needs of all patients served through support of the nursing process; collects patient care data, reports findings, assists team to meet patient care goals. 

3. As directed by licensed staff, provides care in a manner that is safe and sensitive to the rights of each patient.

4. Monitor's patient reaction and compliance with care plan. Recognizes and accurately reports relevant patient information to licensed staff 

5. Utilizes time, equipment, and supplies in an efficient safe manner. Completes all assigned duties during designated shift. 

6. Determines priorities in workload

7. Completes care activities in an accurate and timely manner.

8. As directed, provides personal care assistance to patients: 

a. Personal hygiene – bathing, bathroom/bedpan, 

b. Self-care activities - dressing

c. Ambulation & exercise

d. Eating

e. Transport

9. As directed, plans and prepares meals ensuring nutritional offerings comply with prescribed meal plan.  Assists patient to eat if necessary, ensuring patient safety. 

10. Performs household services to promote patient's independence/ self-care and provide a clean and safe environment. 

11. Recognizes unusual conditions outside of his/her expertise and seeks guidance of case manager.

12. Participates in educational programs, attends in-service activities and workshops, attends staff meetings as required

13. Manages the ordering and organization of the clinical supplies for the department. 

14. Responds to the needs of the department by performing other duties, as necessary.
