PURPOSE OF THIS POSITION

The Home Health Manager is responsible for managing the home health care provided to Bridge Clients. The Home Health Manager oversees the planning, coordination, and delivery of direct care provided by the Home Health nursing and therapy teams through care plans, OASIS, scheduling, document review, and direct supervision via the Therapy Supervisor.  This position reports to the Director of Operations.

JOB DUTIES/RESPONSIBILITIES

Duty 1: Demonstrates the knowledge and skill to mentor the Home Health Care Team in providing consistent quality patient care to those being referred to or receiving Home Health Care services.

Duty 2: Coordinates schedules of all clinicians in relation to the Plan of Treatment. Assigns primary patients and/or visits per Bridge Home Health and Hospice Guidelines.

Duty 3: Manages the clinicians in performance of their daily duties and assists clinicians with assessment/intervention/evaluation skills.

Duty 4: Plans and participates in the orientation of new associates and in the on-going education of current associates.

Duty 5: Ensures staff's documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.

Duty 6: Participates in Administrator On-Call duties as assigned.

Duty 7: Coordinates with the quality team and Clinical Educator to provide guidance to clinicians to enable them to consistently provide optimal patient care within budget constraints. Responsible to upper management in the use of all resources budgeted. Utilizes the skills of the Therapy Supervisor and Clinical Educator. 

Duty 8: Facilitates Interdisciplinary Team meetings and communication/collaboration within the Home Health Care Teams.

Duty 9: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.

Duty 10: Complies with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance to include OASIS.

Duty 11: Other duties as assigned.

REQUIRED QUALIFICATIONS

  • Current state licensure as a Registered Nurse (RN)

  • 1-2 years of general field experience.

  • Commitment to and knowledge of the philosophy, mission and goals of the agency and standards and governing regulations.

  • A valid driver's license is required (if you do not have a valid Ohio driver's license you must obtain one within 30 days of your residency in the state).  You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.

  • Positive service-oriented interpersonal and communication skills.

  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status.  Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures.

  • Homecare work experience.

PHYSICAL DEMANDS

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
