Summary:

The Diabetes Nurse Educator demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Extraordinary Care for Every Generation.

The Diabetes Nurse Educator in conjunction with other health professions is responsible for assessing, planning, implementing and evaluating the care and education provided to patients with diabetes. The Nurse Educator is responsible for working with the leadership team including the Diabetes Coordinator to accomplish goals based on policies and procedures, AADE standards and other regulatory agencies.

Responsibilities:

Develops education plans collaboratively with patient and family evaluating individual needs (i.e. present knowledge, past experience cultural and religious beliefs, support systems, etc.)

Assesses patient's readiness to learn, and implement appropriate plan of care Educates diabetes self-management skills utilizing the 10 content areas as outlined in the National Standards of Diabetes Self-Management Education

Empowers the patient with diabetes to make necessary changes

Provides education on an individual and/or group class setting

Monitors patient's progress and responsible for communicating recommended changes in care to PCP

This includes adjusting insulin, making pump changes and evaluating blood sugar results under the doctor's orders

Demonstrates adherence to safety and legal requirements established at Covenant HealthCare and department level with an emphasis on minimizing risk to patient and staff Involved in the decision making for overall department operations

Works closely with leadership team

Works with leadership team and peers to implement and evaluate patient care progress to consistently maintain the current standards of diabetes education

Helps develop systems to monitor customer satisfaction and designated outcomes

Participates in community initiatives to support the needs of current customers and market the program to potential customers

Remains current with the knowledge and standards of the specialty of diabetes self-management and demonstrates competency in clinical judgment and technical skills

Adheres to all regulatory agencies and standards

Identifies self-learning needs and seeks way to enhance current competency

Recognizes and responds to new research/literature finding that have an impact on patient care

Other information:

EDUCATION/EXPERIENCE

Required:Graduate of an accredited school of nursing and licensed to practice as an RN in the State of Michigan

Preferred: Bachelor of Science in Nursing (BSN), or actively pursuing

Credentials Certified Diabetes Educator (CDE) or actively working towards it (exam should be taken once eligible)

Experience Two to three years experience in a clinical or education setting

KNOWLEDGE/SKILLS/ABILITIES

Demonstrates excellent customer service skills

Computer competency, including Microsoft Office and Outlook

Strong organization skills and attention to detail

Excellent problem solving and critical thinking skills

Strong verbal, written and interpersonal communication Knowledge of electronic record preferred Ability to analyze and interpret situations and information Ability to work as a team member, handle stressful situations, follow directions, meet deadlines

Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
