Summary:
The Hospital Medical Social Worker demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation. This individual is responsible and available on an ‘as needed' basis for psychosocial assessment, counseling, intervention and complex discharge planning for patients and families.

The Social Worker works independently in a professional capacity to assess patients who present with psychosocial concerns. Referrals may come from physicians, nurses, Case Managers or others on the treatment team. Families and patients may also request services directly. The scope of the Hospital Medical Social Worker's assignment may include unit specific or illness- specific roles. The Hospital Medical Social Worker provides intervention to address and ameliorate identified concerns and may also make recommendations to the Multidisciplinary Care team on issues of a psychosocial nature. Intervention modalities may include individual, family and group psychotherapeutic interventions, crisis intervention, bereavement counseling, education, interventions to restore the capacity for social functioning, case management, referral, coordination of services and advocacy. The Hospital Medical Social Worker may also provide education and training activities for patients, families or staff on areas of psychosocial concern.

When illness or injury necessitates life changes, the Social Worker intervenes by assisting families to make decisions, access resources and cope with the resultant life adjustments. When patient or family difficulties interfere with treatment, discharge planning, informed consent or physician/ patient communications, the Social Worker is available to assist with assessing the barriers and through therapeutic relationship building, makes recommendations or interventions to address and resolve the barrier. The Medical Social Worker assists patients and families to cope with hospitalization and may help with tangible needs that impact on their ability to support the patient or contribute to a safe discharge plan for the patient. This may include assisting families with lodging or basic needs, or helping families to access appropriate resources for these needs. The Social Worker provides consultation and recommendations on psychosocial issues that affect the daily challenges to service provision at the request of Physicians, Case Management Specialists, Unit Managers, Nursing staff and other members of the healthcare team and through on-going relationship building with other disciplines and on their assigned Units.

The Social Worker must be able to assess the plan of care for patients in the following categories:
Neonatal/Newborn (birth – 30 days) - PEDS, NICU, OB, ECC
Infant (30 days – 1 year) - PEDS, NICU, ECC
Pediatric (1 – 12 years) - PEDS, ECC
Adolescent (13 – 17 years) - PESD, OB, M/S, CC, ECC
Adult (18 – 64 years) - OB, M/S, CC, ECC

Responsibilities:
Demonstrates excellent customer service.
Contributes to organization success targets for patient satisfaction by meeting the Clinical Resource Management Expectations for Customer Satisfaction.
Contributes to organization success targets for net operating margin by responding in a timely and effective manner to Social Work referrals.
Ensures the availability of accurate and timely information through documentation and effective communication with other staff.
Demonstrates age specific competency in the selected age groups: newborn, infant, pediatric, child, adolescent, adult, and geriatric.
On a referral basis, provides psychosocial assessment of the identified patient/ family. Identifies problems, needs and strengths. Collaborates with patient/ family to develop a plan to address identified concerns.
Collaborates with the Physician, Case Manager and Nursing on cases with complex pscyhosocial needs or social barriers to appropriate and timely discharge.
Based upon the psychosocial assessment and multidisciplinary care planning, provide psychosocial intervention to patients and families which may include individual, family and group psychotherapeutic interventions, crisis intervention, bereavement counseling, education, interventions to restore the capacity for social functioning, case management, referral, coordination of services and advocacy.
Makes recommendations to the Physician, Unit Manager, and / or Multidisciplinary Team on patient care, education or discharge needs based upon assessment and findings.
Arranges placement in Psychiatric, Substance Abuse or specialized placement facilities.
Facilitates specialized resources and assists patients with accessing these resources.
Facilitates and or participates in multidisciplinary care conferences as needed for complex cases.
Attends Rounding on the Units, as assigned and according to availability.
Develops effective working relationships within the assigned units to improve communications and advocate for patients.
Serve as liaison between patients, families and community resource agencies. Develops and maintains knowledge of relevant community resources.
Documents psychosocial assessment, findings, recommendations, interventions and outcomes in the medical record according to established policy and procedure.
The Social Worker may follow up with patients post discharge to assess outcomes, initiate appropriate interventions, and identify opportunities for improvements in care process.
Maintains patient confidentiality according to all applicable laws, policies and regulations.
Serves as a patient and family advocate. Is aware of the patient's diagnosis, prognosis, and insurance or other resources available.
Participates in Social Work and Department meetings.
Participates and assists with Ethics Committee referrals on assigned unit.
Engage in continuing education to improve knowledge, skills and maintain CEU's for licensure.
Provides consultation to Units and programs on issues of a psychosocial nature, including practice and policy development.
Demonstrates excellent communication skills, negotiation skills, diplomacy and assertiveness.
Builds and nurtures professional, effective relationships with all members of the Healthcare team.
Manages conflict effectively, striving for win-win outcomes.
Serves as a patient and family advocate. Is aware of the patient's diagnosis, prognosis and insurance benefits available. Works to maximize positive outcomes.
Serves as a resource to the health care team related to discharge planning, utilization management, case management and patient pathways specifically on issues of a psychosocial nature.

Other information:
EDUCATION/EXPERIENCE
Masters degree in Social Work from an accredited school of social work is required.
Current master's level clinical license with State of Michigan required or able to obtain within 6 months of hire (Licensed or Limited License)
Minimum of three years experience in a Healthcare facility or related community agency preferred.
Has exceptional understanding of treatment regimens for complex psychosocial issues
Demonstrated ability to work as a member of a team where assignments must be coordinated with peers.
