Physical Therapist - Per Diem 
Wayne County Hospital 
Monticello, KY 

As a Per Diem, nonexempt CPS Solutions, LLC Physical Therapist for Wayne County Hospital's Occupational Rehab Department in Monticello, Kentucky,you will be responsible for the evaluation, planning, and directing of treatment as prescribed by a licensed physician to restore function and prevent disability following injury or disease. You will participate in operational aspects of the department, maintain performance improvement activities within the department and participate in CQI activities. 

Schedule: This position can work up to 19 hours per week, and will provide coverage "as needed". This position has no regularly scheduled hours.

About the Facility:  Wayne County Hospital is a 25-bed Critical Access Hospital in Monticello, Kentucky. It is located about 30 minutes southwest of Somerset, Kentucky and about 5-8 miles from the shores of Lake Cumberland. CPS Solutions, LLC has partnered with the hospital to manage the day-to-day operations of the Occupational Rehabilitation department. This position is an employee ofCPS Solutions, LLC.

About the Rehab Department:  Hours of operation are Monday through Friday from 8:00am-6:00pm. 

Required Qualifications: 

  • Graduate of an accredited College or School of Physical Therapy
  • Bachelor's Degree required, Masters's degree preferred  ​  ​
  • Must be licensed as a Physical Therapist in Kentucky
  • Orthopedic sports medicine is preferred but not required 

Hospital Requirements: 

  • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months 
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity   
  • Varicella - (2) documented doses or A Blood Titer proving immunity   
  • (Flu) Influenza-acceptance or declination for hires between Oct 1st-April 30th   
  • Respirator FIT Mask Test - Onsite 
  • COVID vaccination (required)
