A Day in the Life of a Care Manager - RN or LSW
The Care Manager-RN role is integral in enhancing patient outcomes by managing complex health issues through thorough assessments, developing detailed discharge plans, and facilitating treatment across various settings. This position requires close partnership with doctors, nurses, patients, and families to optimize health service delivery and ensuring all medical, psychological, and social needs are met efficiently. The Care Manager is responsible for Utilization Review, ensuring that medical treatments provided are necessary and within the best cost parameters while adhering to quality standards. Collaboration with billing offices and insurance providers is essential to address coverage issues and secure patient benefits. Typical responsibilities include ongoing patient monitoring, advocating during care transitions, and contributing to team-led patient support initiatives.

Qualifications
Education and Experience Required
- Graduate of an accredited program of Social Work or a Bachelor's degree in Social Work from a nationally or regionally accredited college or university
- RN to have Heart Association Basic Life Support (BLS) within 30 days of hire
- RN License to practice in the State of Minnesota or Social Workers license in the State of Minnesota
- MSW
- Previous Care Management and Utilization Review experience
- Three years clinical experience in the acute care setting
- Three years of experience as a social worker in the acute care setting
- If an RN, Bachelor of Science/Art in Nursing
The pay range listed above is the estimated starting pay range for this role. Actual pay rate offered depends on many factors including each individual employee's experience level, preferred education/certifications, along with market conditions.
