Description

Under direction from the CMO, the candidate provides patient care in an outpatient setting by diagnosing and treating disorders and diseases, performing patient exams, ordering or conducting diagnostic testing, and developing ongoing treatment and disease-management plans. In this role, the candidate will also prescribe medication and coordinate referrals as necessary, conduct early-detection screenings and maintain accurate patient records and charts. Effective communication with patients, physicians, staff, and administration is crucial.

The ideal candidate:

  • Has excellent clinical skills.
  • Is dedicated to delivering high quality, compassionate care to patients.
  • Is licensed to practice medicine in the state of Arizona.
  • Is board certified or board eligible in their specialty.
  • Demonstrates a work history of positive collegial relations with colleagues, support staff, hospital-based providers, administrators and patients.

Duties:
  • EMR: Cerner
  • Patient populations: This is a Dissemination site of UCLA ADC Program, started in the Summer of 2021. Patient population are those with dementia and their caregiver(s).
  • This position will be Alzheimer's and Dementia Care focus.
  • Project volume: 250 patients
  • Office Patients: Patients are seen in the office with some off-site home visits in the surrounding area.
  • Typical work week (hours worked; hospital vs. clinic) - M-F 8hrs a day - Working in both the clinic setting, as well as, off site visits throughout the community to accommodate the patient's needs. Typical patient visit load of 2 new consults a day and f/u visits.
  • Call: Shared call.
Qualifications:
  • Current Board Certification
  • Valid medical license
  • Valid DEA certificate
  • 2-5 years' in geriatrics. Focus on geriatric cognitive diseases preferred.
