Director of Health & Wellness (Registered Nurse/RN) – Discovery Commons Brookfield
Location: Brookfield, WI  

Compensation: $115k – $125k base + 10% bonus

Hours: Monday–Friday, 8:30 AM – 5:00 PM (some flexibility), one weekend MOD shift/month, on-call for clinical oversight

Discovery Management Group is seeking an experienced Director of Health & Wellness to lead the clinical and care services of our Assisted Living and Memory Care community at Discovery Commons Brookfield. Our community is defined by a passionate leadership team and a dedicated core staff whose strong retention reflects their commitment and care. Your Role: Director of Health & Wellness

As the Director of Health & Wellness, you will oversee the overall health and wellness operations of the community. This includes leadership of licensed nurses, care managers, and support staff to ensure that residents receive exceptional clinical care and services. You will play a critical role in compliance, team development, family communication, and maintaining the highest quality of resident outcomes.

Responsibilities

Clinical Leadership & Compliance

  • Ensure compliance with all federal, state, and local clinical regulations, as well as [Manco Placeholder] policies and ownership standards.
  • Oversee assessments, service plans, and resident health documentation in accordance with state requirements.
  • Monitor occurrence trends, implement corrective action, and lead quality improvement initiatives.
  • Maintain communication with resident families regarding changes in care or health status.
  • Partner with the Executive Director and department heads to align resident health and safety goals with community operations.

Resident Care & Services

  • Lead clinical staff in providing exceptional care, including wellness checks, medication management, and service plan updates.
  • Ensure resident service plans are updated and accurate to reflect current health status.
  • Partner with pharmacy consultants and physicians to optimize resident care.
  • Support resident/family education and engagement in the care process.

Team Leadership & Development

  • Hire, train, schedule, and supervise nurses, care managers, and other clinical team members.
  • Provide ongoing coaching, feedback, and development opportunities for staff.
  • Promote accountability, teamwork, and a culture of resident-first service.
  • Ensure adequate staffing levels and clinical supplies to meet community needs.

Qualifications

  • Registered Nurse (RN) license in good standing in the current state is required; RN preferred.
  • Must have prior Assisted Living and/or CBRF experience.
  • Minimum 1 year of nursing leadership experience.
  • BLS Certification required.
  • Strong leadership skills with the ability to manage, motivate, and develop a care team.
  • Excellent communication, problem-solving, and decision-making skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Participation in on-call and Manager on Duty (MOD) is required.
