We are currently seeking a motivated and compassionate Full-Time Licensed Practical Nurse (LPN) to join our dynamic team.
**Position Overview:**
As a Full-Time LPN at Baker Place, you will function as assistant to the Director of Health and Wellness.
You will play a critical role in delivering excellent patient care in our Assisted Living facility. Your responsibilities will include working alongside our Registered Nurse to provide both direct and indirect patient care, education, and support.
Must be able to work 3 weekdays per week 8:00am-4:30pm, plus every other weekend 6:00am-2:00pm on the medication cart. Ability to pick up shifts if necessary.Weekdays consist of assisting RN with chart filing, calling physicians, setting up labs/podiatry/eye appointment, etc. Assisting with calls, resident and family questions and concerns, incident reporting, education, and other assigned tasks.
Weekends consist of overseeing clinical duties of HHA/CNA, running the medication cart, administering medications, cart reviews, pharmacy follow-ups and other assigned tasks.
**Other Responsibilities:**
- Provide basic nursing care and assist in the daily living activities of patients.
- Monitor and record vital signs, and document patient health information.
- Administer medications as prescribed by physicians.
- Assist in wound care, dressing changes, and personal hygiene tasks.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Communicate effectively with patients and their families regarding treatment plans and care instructions.
- Participate in patient assessments and contribute to ongoing evaluations.
- Maintain a clean, safe, and organized environment.
- Adhere to infection control protocols and safety guidelines.
- Stay current with nursing best practices and continuing education requirements.
**Qualifications:**
- Current and valid LPN license in the state of NJ.
- Completion of an accredited practical nursing program.
- Previous experience in a healthcare setting preferred.
- Excellent communication and interpersonal skills.
- Ability to work well under pressure and handle multiple tasks simultaneously.
- Strong attention to detail and commitment to providing high-quality patient care.
- Basic computer skills and experience with electronic health records (EHR) preferred.
