Job Summary:
The Transition Assistant works closely with members of the care team and central team, as well as physicians and staff in coordinating and communicating gaps in quality care metrics for patients. They will assist with low risk patient navigation and preventative screening compliance. The Transition Assistant audits clinical data and assists in educating providers and staff. This promotes workflows that support quality patient care and compliance with documentation and clinical outcome metrics.

Job Essentials: The ideal candidate will meet the following requirements and competencies.
- Minimum of 3 years' experience in a fast-paced medical office setting with a leadership skill set
- Knowledge of Quality Measures
- Must Possess strong interpersonal and communication skills in order to interact effectively with patients and families, physicians and office staff
- Ability and willingness to identify operational and/or staff and provider issues and elevate them for resolution
- Demonstrated problem-solving ability and experience
- Must have working knowledge of medical terminology and computer literacy, to include experience with practice management software
- Experience with Epic electronic medical records software, preferred
- A team player who has demonstrated a commitment to his or her current position and show leadership potential
- A high energy, self-starter with a strong work ethic who is proactive and anticipatory rather than reactive, and accepts delegation enthusiastically
Education Requirements:
- High School Diploma or equivalent required.
- Associate's degree in health related field preferred.
Licenses, Registrations, and Certification:
- Current driver's license required and access to a vehicle.
