Palliative Care Outpatient Medical Director

Hackensack Meridian Health (HMH) is seeking a qualified palliative care physician with 5+ years of leadership experience to fill the role of Outpatient Medical Director, Palliative Care. The successful candidate will have overall accountability for clinical outcomes of palliative care delivered in the home and outpatient palliative care of Hackensack Meridian.

Position Responsibilities

  • Dedicate a portion of his/her time to providing direct palliative care consultations in various settings.
  • Ensure standardization of process and protocols and be an integral part of a network-wide palliative care harmonization workgroup.
  • Supervise advanced practice and registered nurses, social workers, and work with administrative team members to provide and coordinate medical care with the patient's community physicians.
  • Provide support and direction for clinician recruitment, training, and deployment of a value-driven workforce aligned with Hackensack Meridian Health's Vision and Mission.
  • Grow a collaborative program and lead a robust clinical program and exhibit a commitment to clinical excellence and innovation
  • Provide oversight for an interdisciplinary care team providing home - based palliative care for patients in their home, as well as for interdisciplinary care teams providing palliative care in the subacute, long-term care, and ambulatory practice(s) settings, as indicated
  • In partnership with other physician leaders, develop education plans, prepare curriculum and present on educational topics such as serious illness communication, advanced illness management and symptom management, among others
  • Assist in the creation of palliative care fellowship

Professional Skills

  • MD/DO, Board-certified in palliative medicine
  • Ability to assess, evaluate, plan, and implement care in complex and life limiting situations
  • Capable of offering specialized knowledge and skill to all members of the health care staff
  • Team oriented and able to provide personalized patient care as well as family support and demonstrate clinical outcomes
