Description

Complete Care Physicians specialize in the care of adult members with high needs due to a complex array of medical, social, psychological, and behavioral factors. These Primary Care doctors focus on matching care to the needs of the individual patient using an interdisciplinary approach focused on increased continuity access, panel management, and care coordination. A comprehensive team of healthcare professionals will ensure the well-being of members by handling a range of services including on-site and virtual primary care, chronic and acute care management, coordination with relevant specialists, and after-hours call coverage. Experience and/or formal training in complex care, geriatrics, and team-based approaches is preferred.
