We are seeking a board certified/board eligible Palliative Care / Geriatric physician. This position will be 80% palliative care and 20% geriatrics. This position will be based at our Kaiser Permanente Gwinnett Comprehensive Medical Center located in Duluth, Georgia.

For palliative care, this provider will be an addition to the palliative care team in caring for Kaiser Permanente (KP) Georgia members with potentially life limiting illnesses. The primary focus is on outpatient care, providing assessments, treatment and recommendations. The physician will work closely with the teams nurses, chaplains, social workers, clinical pharmacy specialist, and other team members. Visits with members will primarily occur in KP medical offices and, telephonically, but may take place in members homes as appropriate. Collaborating with the members other physicians, the physician in this position will treat and/or recommend care for members' medical problems and chronic illnesses with emphasis on patients quality of life and functional ability. This will include counseling on advance directives, physical and psychosocial concerns, end of life care, and other topics as appropriate.

For geriatrics, the provider will care for members 65 and older and those with Medicare needing special expertise of the team. The primary focus is on outpatient care, providing assessments, treatment and recommendations for members with conditions common in older patients such as memory loss and falls. This physician will work closely with the teams geriatricians, nurse care managers, nurse practitioner, social workers, clinical pharmacy specialist, and geriatric psychiatrist. Visits with patients will primarily occur in Kaiser Permanente (KP) medical offices, telephonically, and via home visits, but may occasionally include visits in skilled nursing facilities. Complementing the primary care physician (PCP), the physician in this position will treat and/or recommend care for patients' medical problems and chronic illnesses with emphasis on patients quality of life and functional ability, including addressing home safety and counseling on end-of-life care and other topics as appropriate.

Ideal candidate must be MD/DO, post-graduate residency training, and board certification in internal medicine or family medicine, fellowship and board certification in geriatric medicine and palliative care medicine, geriatric medicine (for internal medicine physicians) or certificate of added qualifications (CAQ) in geriatric medicine (for family medicine physicians). Candidate must have one year post residency experience in caring for palliative care patients in an outpatient and/or facility setting.

Minimum Education and Experience Requirements:

One-year post-residency experience in caring for older patients in the outpatient and/or facility setting

Demonstrated ability to effectively communicate with older patients and older patients with potentially life-limiting illness, their family members/caregivers, and other physician and non-physician health professionals

Ability and willingness to travel to various locations for patient care

Preferred Education and Experience:

Two years post-residency experience in providing palliative care services to patients in the outpatient and/or facility setting

Experience working as part of an interdisciplinary team

Experience in a skilled nursing/custodial care setting

Experience in performing home visits

Required Licensure, Certification, Registration or Designation:

GA state licensure

DEA registration
