Position Summary Under general direction, protects, promotes, and optimizes health and abilities. Prevents illness and injury. Alleviates suffering through the diagnosis and treatment. Advocates for patients and their families. Ensures all actions taken in carrying out responsibilities support patient centered care. Position Responsibilities Unit Specific Position Responsibilities Provides in-home visits to home health clients within the agency#s service area. Conducts OASIS assessment and documents information in the automated clinical record. Serves as case manager for home care clients using assessment data to develop a plan of care based on evaluation of identified problems. Educates patients and families for home care and discharge planning needs. Provides appropriate transfer or discharge instructions/information to patient and family. Participates in after hours on-call rotation responding to client needs via phone assessment with in-home visit if appropriate. Plans and participates in public health activities as assigned. Qualifications, Knowledge # Experience Required Qualifications (Including any licensure, certification, education): Licensed as a Registered Nurse and authorized by the State of Iowa to practice nursing or has active multistate license approved by the Nurse Licensure Compact. Achievement of Baccalaureate degree in nursing within 6 years of hire if hired after January 1, 2014. Basic Life Support#(BLS)#training, or within 90 days of hire and must maintain throughout employment. Mandatory Reporter training or within 90 days of hire and must maintain throughout employment. Two years experience in medical surgical nursing, home health or hospice. Possession of a current valid driver#s license and must maintain eligibility for coverage under Mary Greeley Medical Center#s insurance policy. Organizational Requirements: Maintain stroke education per regulatory requirements. Preferred Qualifications: Baccalaureate degree in nursing. Professional certification in nursing specialty. Certification in Health Coaching Required Knowledge, Skills # Experience: Proficient in communication skills. Ability to pursue and foster a sense of team and partnership across all disciplines. Ability to communicate effectively, in English, with diverse population groups. Ability to be self-directed, flexible, and able to adapt to change. Ability to communicate effectively in a professional and tactful manner with physicians, staff, patients and/or their representatives. Ability to read and understand chart documentation. Ability to demonstrate basic computer skills. Ability to read, write and understand the English language. Knowledge of theory, principles, techniques and practice of professional nursing; of scientific principles and nursing research; of preparation, administration and actions of medications; of treatments and medical procedures; of counseling and teaching techniques; of medical equipment as it relates to patient care# and ability to operate and handle many kinds of complex equipment.
