General Summary of Position
The Therapy Care Manager plays a vital role in guiding the field training experience for newly hired clinicians mentoring them throughout their first year to promote engagement retention and the development of clinical expertise. This role ensures high-quality care delivery by providing clinical support training guidance and fostering sound decision-making among field staff. Key responsibilities include performing patient care and case management as needed in collaboration with the Director of Therapy Services and the Clinical Operations Manager. Additional duties involve supporting quality care initiatives case conferencing staff coordination and managing clinical workflows.In partnership with the education quality and operations teams the Therapy Care Manager assists the Professional Training Specialist in developing the clinical orientation programs for new hires facilitates their integration into the team and serves as a field trainer to promote best practices. This role actively contributes to quality assurance through joint visits chart audits participation in mock surveys and supporting fall occurrence resolution.Serving as a knowledgeable and collaborative resource this role provides clinical support to field staff under the supervision of the Director of Therapy Services AVP of Operations and in collaboration with Professional Training Specialist and Clinical Operations Manager within assigned teams.
Primary Duties and Responsibilities
Education
- Bachelor's degree in Physical Therapy required or
- Master's degree in Physical Therapy (MPT) required or
- Doctoral degree in Physical Therapy (DPT) required
- 1-2 years Experience in a certified home care or home hospice program required
- PT - Physical Therapist License to practice Physical Therapy in all jurisdictions in which the employee provides services to patients the District of Columbia Maryland or Virginia or any combination based on work location(s) Upon Hire required and
- DL - Valid State Driver's License in good standing and a dependable vehicle for transportation Upon Hire required and
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
- Additional unit/specialty certifications may vary by department or business unit.
- Ability to case manage.
- Organization skills.
- Excellent verbal and written communication skills.
- Basic computer skills preferred.
- Knowledge of home equipment home care reimbursement and regulations governing PTA's LPN and HHA's.
This position has a hiring range of
USD $89,065.00 - USD $162,801.00 /Yr.
