General Summary of Position

MedStar Health is looking for a Case Manager Social Worker to join our team at MedStar National Rehabilitation Hospital! 

The Case Manager works with patients, families, caregivers, the rehabilitation team, and insurance provider(s) to ensure a coordinated rehabilitation program and maximize patient outcomes. The Case Manager also works with the patient, caregiver, and team to coordinate a safe and effective continued care plan.

Primary Duties: 

  • Conducts a comprehensive psychosocial interview with patient(s) and family members. Provides conflict management and mediation with the patient, their family, and/or caregivers.  

  • Acts as a liaison between patient, family, caregivers, and treatment team to ensure that everyone understands patient's perceived needs and concerns. Collaborates with the multidisciplinary rehab team, patients, and significant others to develop an appropriate team treatment plan.  

  • Manages healthcare resources effectively to include accurate reporting to insurance providers and advocacy for patients' continued care needs. Advises the physician, team, and UR team of potential denials (i.e., Medicare and private insurance companies).  

  • Facilitates daily interdisciplinary team conferences and family conferences. Participates in program development and process improvement tasks within the department.  

  • Makes referrals to appropriate community agencies to provide patients and families with the resources needed to maximize independence and wellness. Facilitates Skilled Nursing Facilities placement and coordinate Home Care referrals. 

 

Qualifications:  

  • Master's degree in social work. 

  • Licensed as a Social Worker in the District of Columbia or Certified Case Manager. 

  • Experience in rehabilitation care or in case management with demonstration of knowledge of rehabilitation preferred. 

  • Experience with Medicare, Medicaid, and private insurance companies preferred.
