General Summary of Position

MedStar Health is looking for an LCSW-C/LICSW Inpatient Case Manager to join our team at MedStar Washington Hospital Center.

The LCSW-C/LICSW LICSW serves as a member of the Case Management Team. Facilitates the delivery of quality, cost effective, patient-centered care from pre-admission through post-discharge timeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay working collaboratively with the multidisciplinary team both internal and external to the organization, promotes effective case management and utilization of resources facilitating the continuum of care, and works to achieve optimal clinical and resource outcomes for the acute, and posthospital phases of care.

Primary Duties:  

  • Collaborates with the multidisciplinary healthcare team to assess patient needs, develop and coordinate individualized plans of care, and facilitate safe, timely discharges.
  • Conducts psychosocial and socioeconomic assessments, provides clinical evaluations for behavioral health concerns, and connects patients with appropriate community and social service resources
  • Facilitates communication between patients, families, and healthcare providers to address social, emotional, cultural, and financial barriers to care..
  • Documents assessments, referrals, and interventions accurately while maintaining compliance with departmental policies, quality standards, and regulatory requirements.
  • Participates in quality improvement initiatives, meetings, and professional development to enhance care management effectiveness and departmental performance.

Qualifications: 

  • Master's degree in social work from a school accredited by the Council on Social Work education (Required)
  • 1-2 years Experience in social work, preferable in a hospital setting (Preferred)
  • 1-2 years Experience in care/case (Preferred)

License/Certifications:

  • LCSW-C - Licensed Certified Social Worker - Clinical License in the District of Columbia or the State of Maryland depending on work location OR LICSW - Licensed Independent Clinical Social Worker License in the District of Columbia or the State of Maryland depending on work location (Required)
  • CCM - Certified Case Manager (Preferred)


This position has a hiring range of $74,214 - $134,596
