RNs support care plans for each patient and then help patients and their caregivers achieve progress toward their goals for improved health and independence. The homecare team includes nurses, physical and occupational therapists, speech and language pathologists, medical social workers, and home health aides who are dedicated to meeting patient needs 24/7/365.

Provides direct and indirect patient care services under the supervision of a registered nurse.
Position Details:
§Develops and documents individualized plan of care with specific care interventions and expected measurable patient outcomes.
§Evaluates progress toward goals and revises plan of care as needed.
§Establishes, documents, and implements personalized teaching plans for patients and their caregivers to help them achieve independence at home.
§Performs nursing procedures consistent with homecare protocols. Interventions reflect knowledge of standard homecare supplies and equipment used in patient care.
§Initiates and participates in regularly scheduled case conferences, which include all disciplines. Includes patient and family in mutual goal setting and care plan revision.
§Maintains necessary clinical records, collects data and prepares reports on activities.
§Communicates regularly with the patient's physician, home healthcare team, and other key members of the care team.

Primary Duties and Responsibilities
Minimum Qualifications
Education
- Vocational/Technical Diploma Graduation from a diploma program in Nursing required or
- Associate's degree in Nursing required and
- Bachelor's degree in Nursing (BSN) preferred
Experience
- 3-4 years RN experience (ICU, CCU, Med-Surg, Oncology, Gerontology) within last 5 years required
- 1-2 years Home health nursing experience preferred
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Valid RN license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s) Upon Hire required and
- CPR - Cardiac Pulmonary Resuscitation Upon Hire required and
- DL - Valid State Driver's License in good standing Upon Hire required
Knowledge, Skills, and Abilities
- IV therapy skills.
- Demonstrated comprehensive knowledge of and technical skills in home care case management and OASIS documentation.
- Strong organizational skills.
- Excellent verbal and written communication skills.
- Basic computer skills preferred.
- Smart Phone experience.
