General Summary of Position
The Social Worker Team Lead Inpatient Case Manager provides patient/family interventions to facilitate quality patient care and timely discharge planning. Coordinates with physicians, nurses, Clinical Resource Management (CRM) Case Managers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of post acute care and discharge planning. Using a variety of treatment modalities, Senior CSWS assists patients and families in the resolution of social, financial and emotional problems related to illness, health care and rehabilitation. Assists to effectively and efficiently provide complete comprehensive discharge plans to decrease LOS by having services available in a timely manner. Provides additional care for the psychiatric, ETOH/drug abuse, rape, neglected adult and child, and self-pay patient; and the highest possible care for planning a safe discharge in a timely manner and assists with projections for discharge and IDRs daily. This position includes meeting the needs and providing services to all age groups-infancy through geriatrics, as well as providing services for the culturally diverse population MedStar Washington Hospital Center serves.

Primary Duties:
· Conducts psychosocial assessments and develops individualized care plans in collaboration with the interdisciplinary team to address patient and family needs.
· Coordinates safe and timely discharge planning, identifying and resolving barriers to transition across the continuum of care.
· Provides counseling and emotional support for patients and families facing illness, crisis, bereavement, or major life changes.
· Leads and participates in care team meetings, legal processes (e.g., guardianship), and referrals for abuse/neglect cases as appropriate.
· Supervises social work colleagues and students, serving as a preceptor and field instructor while maintaining accurate documentation and contributing to departmental initiatives.
Qualifications:
· 1-2 years progressively more responsible exp in a medical setting (Required)
· 1-2 years employment as a Clinical Social Worker at MedStar Washington Hospital Center (Preferred)
· Master's degree in social work Required.
· 1-2 years' experience Preferred.
· Home health care Preferred.
License/Certifications:
· LGSW- Licensed Graduate Social Worker in the District of Columbia (Required)
· Lic Independent Clin Soc Work – LICSW in the District of Columbia (Preferred)
This position has a hiring range of $72,758 - $130,041
