M-F, full time, 40 Hours per Week 

Overview:

  • Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met, and quality care provided in accordance with Federal, State, JCAHO, and program guidelines.
  • Manages the activities of the multi-disciplinary team providing care to patient.
  • Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
  • Ensures that all visits scheduled and provided are authorized and covered by physician orders
  • Assesses and evaluates ongoing patient care needs and communicates those needs to the physician.
  • Demonstrates sound judgement and independent problem-solving skills in order to initiate appropriate intervention with regard to patients psychological and/or physical impairment
  • Conducts admission and follow up visits according to the plan of care.
  • Facilitates the care of the patient in the home setting by utilizing appropriate community resources, counseling and teaching patient/family and advocating on behalf of the patient
  • Ensures patient visits are schedule including any anticipated weekend visits.
  • Informs nursing scheduler daily of any schedule changes and communicates caseload and patient care issues to Nurse Manager
  • Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver, and members of the health care team.
  • Interprets agency policy to patients/families- Ensures that all visits made to patients have prior authorizations, if required by the managed care insurance company.
  • Works collaboratively with the Quality Specialists, providing required information according to schedule
  • Shares professional knowledge and opinions with administration through supervisor regarding program planning, policy development, and priority setting for services- Keeps knowledge of professional practice current and acquires new information and skills as needed by attendance at workshops, courses, and in-service education
  • Communicates pertinent patient information to Nurse Manager and coordinates with involved disciplines for accurate OASIS scoring.
  • Review SHP alerts and follows up timely if corrections are needed.

Case Manager Qualifications:

  • Education: Graduate of an accredited school of nursing
  • Licensure: Current Arkansas license as a registered nurse or LPN
  • Certifications: Current certification in CPR
  • Experience: 2 years of nursing experience
