Overview:

Home Health is a dedicated unit offering medical care directly to patients in their own homes, primarily focusing on post-hospitalization recovery, rehabilitation, and disease management. This unit serves a diverse group of patients, from those recovering from surgeries to individuals managing chronic illnesses. Services include wound care, post-operative follow-ups, and therapy sessions. Home Health clinicians have a unique role, traveling to various patient homes throughout the day, ensuring optimal care and aiding in the transition from hospital to home. Their role emphasizes patient education, promoting independence, and ensuring a smooth recovery process. Clinicians in this unit typically work varied hours, allowing for a balance of clinical care and patient interaction. No in-hospital duties.

Qualifications:

• Required Education: Nursing Degree from an accredited school
• License: Oklahoma RN licensure
• Certifications: Basic Life Support certification through the American Heart Association
• Experience: At least 1 year of Home Health/Hospice as a RN. 
• Preferred Certifications: ACLS
• Other: Demonstrates competence by completion of Universal Skills Inventory and Unit Specific Skills Check List within first three months of employment and undated annually.
