Job Summary

The Senior Safety Partner is an integral part of the Clinical Care Experience team and will ensure the Pulse clinical operating system is implemented across the continuum of care. The Senior Safety Partner is responsible for coordinating and implementing a collaborative and proactive patient safety program, promoting a culture of safety and high reliability, and developing, implementing, improving, and evaluating patient safety systems. The position is responsible for supporting continuous safety improvement and ensuring sustained adherence to established Pulse program elements, standard work practices, regulatory requirements, and accreditation standards while partnering with Infection Prevention, Quality, Experience, Advocacy, Accreditation, Clinical Quality Data, and Risk. The Patient Safety Professional is responsible for reviewing Pulse Power reports, performing investigations and risk assessments, and identifying and communicating patterns and trends that will facilitate improvement. Other key responsibilities of the Safety Partner include:
• Serving as a resource for development and implementation of patient safety policies and activities
• Identifying methods for risk identification, reduction of error, and other factors that contribute to adverse outcomes
• Facilitating safety initiatives using a collaborative, proactive approach and connecting with front-line staff and key stakeholders to identify barriers to care and areas of patient safety opportunity
• Providing guidance, partnership and standard work for the design and maintenance of a comprehensive patient safety program
• Communicating evidence-based ideas regarding effective patient safety strategies and techniques
• Working in collaboration with Human Resources, Environmental and Life Safety, and other leaders to develop and assure a culture of safety in the workplace

The position supports the campus/site clinical and administrative leadership team, quality, risk management, accreditation, advocacy, experience, and infection preventionists colleagues, related Subject Matter Experts (SME), stakeholder groups, and ad hoc teams on safety related performance improvement activities. The scope of the role may apply to the ambulatory, acute and/or post-acute care settings.

Accountabilities

  • Provide direct support to the development, implementation, and operation of the Pulse program. Establish standard work and participate in efforts for concurrent monitoring as needed for compliance and results based on new implementations or processes under review. Perform proactive identification and correction of patient safety issues. - 10%
  • Support safety huddles and perform rounds to proactively identify safety risks and hazards and evaluate processes to assure adherence to the Pulse program and standard work processes. Recommend enhancements for improving patient, team member, and environmental safety. Proactively addresses questions or concerns involving safety.  – 10%
  • Complete timely review of Pulse Power submissions by team members and or medical staff for the purpose of identifying patterns or trends. Facilitates opportunities for improvement and support efforts to prevent future events. Work in collaboration with leadership and team members to ensure timely reporting and management of patient safety issues and findings at the operational level. Provide and communicate patient safety trend reports to stakeholders and leaders, in addition to assisting in the development and monitoring of action plans.  - 15%
  • Partner with appropriate SMEs to facilitate failure mode effects analyses and identify high risk processes and systems to proactively promote safety and error prevention. Utilize information from external reporting programs to mitigate risk (i.e. Institute for Safe Medication Practices (ISMP) Medication safety alerts, Emergency Care Research Institute (ECRI), Joint Commission Sentinel events). Facilitate development of actions plans and oversee implementation.   – 5%
  • Facilitates a thorough root cause analysis through the RCA process in the event of a serious safety event or Great Catch using robust process improvement tools and techniques. Facilitate development of strong action plans. Assist operational leaders with implementation of relevant strategies and disseminate lessons learned from RCA process and recommendations across the organization. – 10%
  • Supports deployment of the AHRQ Culture of Safety Survey and data to guide improvement.  – 5%
  • Partner with Infection Prevention, Quality, Experience, Advocacy, Accreditation, Clinical Quality Data, and Risk to integrate and assimilate information obtained from data analysis and operational issues related to risk. Analyze findings and work collaboratively with key stakeholders to support the Pulse program and enhance an organizational culture that supports safety and reduces risk.  – 10%
  • Prepare, provide, and participate in the reporting of patient safety related information to Leadership, Governing Board, and other applicable committees, per schedule and on an as needed basis. Participate in annually reviewing the effectiveness of the Patient Safety program and contribute to organizational goal setting and prioritization of initiatives at least annually. – 10%
  • Contribute to deploying the Pulse program to the organization and support evidenced based practices including High Reliability, Just Culture, and Culture of Safety concepts. Develop and participate in patient safety orientation and educational endeavors, annual recurrent training, one-on-one consultations, and in-service training. – 15%
  •  Serve as a partner on patient safety related subjects. This includes providing direct support to programs that involve patient safety. Participate in the development of administrative and clinical policies regarding patient safety. Work with all stakeholders to implement programs designed to maximize patient safety performance and evaluation as established by the National Quality Forum (NQF) and The Joint Commission (TJC) National Patient Safety Goals (NPSG) and Sentinel Event Alerts. Conduct training and coaching, and develop templates, tools, and processes to facilitate an efficient and effective means for patient safety improvement. Disseminate patient safety best practices and educational materials and communicate evidence-based ideas regarding effective patient safety strategies throughout the organization. – 10%

Supervisory/Management Responsibilities

Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health or affiliate and will have direct reports.

Minimum Requirements

  • Bachelor's Degree - Nursing, healthcare related field, health administration.
  • 7 years - Experience in healthcare with knowledge of practices relating to patient care. Facilitation, Root Cause Analysis, and Process Mapping experience; ability to prioritize an ever changing environment  

Required Certifications, Registrations, Licenses

  • current licensure in related field
  • Certified Professional in Healthcare Risk Management (CPHRM), Associate in Risk Management Certification (ARM) or like certification (e.g., Certified Healthcare Safety Professional (CHSP); Certified Professional in Patient Safety
  • (CPPS)).

Knowledge, Skills or Abilities

  • Initiative and ability to work with limited supervision.
  • Knowledge of federal, state, and local requirements of regulatory and accreditation agencies. Proficiency and demonstrated effectiveness in problem solving, analytical skills, and implementation of new programs.
  •  Knowledge of quality improvement measurement, analysis, and improvement strategies.
  • Strong leadership and interpersonal skills. Highly effective oral and written communication skills to work with all levels of the organization and outside agencies.
