The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. Learn About Our State Supported Living Centers

Performs advanced (senior-level) consultative services and technical assistance community services work serving individuals with intellectual disabilities as they are identified for and transition from a State Supported Living Center into a community setting. Work involves coordinating educational opportunities for individuals with intellectual disabilities, legally authorized representatives, families, staff, community providers and local authorities; identifying obstacles to community transition; and developing action plans. May supervise the work of others. Works under limited supervision, with considerable latitude for the use of initiative and independent judgment.

 

Essential Job Functions (EJFs):

1. Facilitates community placements for individuals residing at SGSSLC into the community programs. Plans and develops the individual's Community Living/Discharge Plan and monitoring activities. Coordinates group home visits for individuals and assists with transportation state-wide.

2. Maintains facility admission data. Coordinates Chapter 55 and Article 46B commitments for forensic evaluations.

3. In the absence of the Admissions Director, chairs the Admissions and Transition Committee to determine appropriateness of services for admission requests. Participates on the Plan of Improvement group or Quality improvement Committee (QIC) to develop and monitor the facility's Plan of Improvement.

4. Serves as Co- lead for provision T of the Department of Justice Settlement Agreement.

5. Coordinates facility practice of the Community Living Options Information Process with the Local Authority. Coordinates staff training on community living options and oversees the provider fair and other community education activities for individuals and staff.

6. Assists IDT's with the temporary consignment of state hospital psychiatric hospitalizations.

7. Attends required meetings, participates in committee functions, and completes required reports and other assignments as provided/requested.

8. Works cooperatively and communicates effectively with other departments as related to the care and treatment of individuals served.

Other duties as assigned including but not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

 

Knowledge, Skills and Abilities (KSAs):

Knowledge of community based services available to individuals.

Knowledge of social services work.

Ability to collect and analyze information to develop a community living plan.

Knowledge of local, state, and federal laws and regulations relevant to program area with the ability to apply them to agency/facility policies and procedures and to convey them to others.

Ability to develop and evaluate administrative policies and procedures, to prepare concise reports, to plan and facilitate meetings and communicate effectively orally and in writing.

Excellent organizational and time management skills.

Excellent communication skills

Demonstrated computer skills to include Word, Excel and Outlook.

Ability to work with people who have developmental disabilities.

Ability to demonstrate commitment to facility mission and workplace/service values.

Ability to develop and provide training to staff, volunteers and individuals.

Ability to travel as needed

Ability to maintain a good working relationship with internal and external supports

Ability to maintain compliance with facility attendance and dependability guidelines

Initial Screening Criteria:

Experience in a field relating to human services. Graduation from an accredited four-year college or university with major course work in human services field. Work experience in a field related to the duties of the position.  

 

Additional Information:

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE).

 

Males between the ages of 18 – 25 must be registered with the Selective Service.

 

All State Supported Living Center Employees are subject to Random Drug Testing.

 

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
