Summary of Job Duties: The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services. The RN Care Manager will work with the individual patient, their families, providers and interdisciplinary care team to develop and implement individual care plans and provide education and self-management support to patients' for their optimal functioning and to improve collaborative coordination of care to reduce waste and inefficiency within the concept of the patient centered medical home. Conducts transitional care outreach to patients within 48 hours of a hospital discharge and assists with effective coordination of services internally and externally. The RN Care Manager achieves targeted outcomes guided by the organization's strategic initiatives, value-based program measures/activities, and as determined within the service line or department. This position will support the ACO population care management and population health activities.

Nursing Process
• Triage, assessment, planning, implementation, and evaluation of nursing care and patient education either by telephone or within the assigned UAMS clinic using the care management model. Conducts care management, including care plans, assessing progress and barriers. Coordinates care and/or follow-up for patients who receive care in inpatient and outpatient facilities.
• Delivers nursing care according to standards as defined by UAMS, American Nurses Association (ANA), AR State Board of Nursing (ASBN), and those set forth by nursing specialty organizations.
• Helps develop patient's Plan of Care (POC) per clinic standards utilizing assessment data.
• Evaluates the progress of patients' toward attainment of desired outcomes.
• Verifies and evaluates care delivered to ensure care is consistent with standards and regulatory requirements.
• Coordinates patient education coordination in collaboration with physician staff.
Education/EBP/Research /QI
• Initiates Performance Improvement, Evidenced Based Practice/Research process.
• Systematically evaluates the quality and effectiveness of nursing practice.
• Supports the integration of current evidence into nursing practices.
• Promotes an environment conducive to positive educational experience for all students and staff.

Communication/Collaboration
• Implements POC using an approach that is interdisciplinary and Patient and Family Centered Care.

Serves as clinic Care Manager and performs all other duties as assigned.

Primary Care RN Care Manager Duties

a) Works in conjunction with all care team members across the continuum to identify and proactively manage complex care needs and close care gaps of the highest risk patients with chronic conditions such as diabetes, uncontrolled hypertension, COPD, CHF, and age-related illnesses within the primary care setting to improve clinical outcomes.
b) Conducts initial and ongoing assessments of patients at high risk that may benefit from care management services. Identifies the high-risk complex patients and works with team members to ensure efficient care coordination for this patient group.
c) Actively manages assigned panel of high-risk chronic care patients utilizing registries and reports. Documents care and interventions appropriately in the EMR.
d) Coordinates care plans and conduct transitional care calls for patients with frequent Emergency Department utilization and those who are recently discharged for inpatient hospital admissions as directed.
e) Participates in patient-centered medical home (PCMH), aids in the development of clinic workflows and collaborates with physicians to develop and maintain current care plans with preventive services and chronic disease management.
f) Identifies and stratifies high risk patients needing care management services to reduce re-hospitalizations in people with chronic illnesses (i.e. uncontrolled Diabetes, HTN, COPD, CHF, depression, out of range BMI and age-related conditions), socioeconomically disadvantaged, and/or the chronically mentally ill.
g) Promotes patient, staff, provider and organizational satisfaction by coordinating and facilitating the delivery of quality and continuity of care while focusing on productivity and the cost effective utilization of resources. Serves as a liaison to providers, patients and families for coordination of services. Work with the providers, pharmacist, social worker, quality improvement coordinator, and other clinical staff to best serve the needs of the patient panel.
h) Provides effective education, self-management support, timely health care delivery/outreach to patients over the phone and in person ensuring all aspects of care plan are met including but not limited to:
(1) Management of transitions in care delivery settings
(2) Review and update medications for reconciliation by provider
(3) Assess and address educational needs associated with chronic disease and prevention
(4) Assess and address any social and psychological determinants that impact disease management
(5) Facilitate adherence to quality metrics determined by quality programs e.g. UAMS, CMS- ACO & Primary Care First, PCMH
(6) Facilitate transfer of information and communication among providers
(7) Educate and encourage adherence to regular primary care provider visits
(8) Serves as an advocate for the patient and family.
i) Use of motivational interviewing to provide 1:1 coaching to patients regarding behavior changes to improve health status, reduce health risks and improve quality of life. Must be able to assess readiness to change and implement actions to assist patients in reaching their goals. Will work with patients to establish health improvement plans, set personalized goals, and support patients in achieving those goals.
j) Develops and supports a practice based care management model by providing on-site and telephonic services i.e. wellness, health coaching, goal setting, self-management techniques, disease management and case management including coordination with community services per Patient Centered Medical Home (PCMH) standards.
k) Facilitates regular patient follow up, education, crisis management, and coordination/support for people at risk or who are vulnerable because of their socioeconomic status and/or level of healthcare literacy who are reluctant to seek treatment due to limited knowledge of their condition and barriers to care.
l) Routinely scans medical literature for updates on clinical practice guidelines, new developments in the management of chronic diseases and communicates and disseminates information to staff.
Qualifications:

Minimum of 3 years RN experience in a clinical environment with current BLS certification.

Certification in Case Management (e.g. CCM, ANCC) or obtain within 1 year of hire date.

Knowledge or experience in PCMH desired.

Valid Arkansas Registered Nursing License
