
Description
Job Duties:
Development, implementation and facilitation of paitent outreach for quality improvement programs.
Review, auditing and reconciliation of quality data reports and related clincal documentation.
Contacting patients to schedule appointments, testing and provide follow-up of results.
Complete responsibilities based upon established protocols and work with mulitple providers.
Requirements:
IN LPN license and BLS required.
Cerner experience preferred.
