Within the team, there are case managers, nurse practitioners and/or psychiatrists, nurses, and others as needed to support an individual's needs. The EOP clinician is the main point of contact for the people on their caseloads, with caseload sizes averaging approximately 60 (changing as agency needs change). At this level of care, EOP clinicians provide direct mental health clinical assessments, individual therapy, and basic case management services to the people on their caseload. EOP clinicians are office based while still providing some community-based support so that they can help people we serve attend to their needs in their natural environments. EOP clinicians transport the people on their caseloads to various locations/agencies as part of linkage to community resources. </span><span data-ccp-props="{"134233117":true,"134233118":true,"201341983":2,"335559739":0,"335559740":300}"> </span></p> <p><strong>EOP Clinicians</strong><strong><span data-contrast="none"> </span></strong><span data-contrast="none">p</span><span data-contrast="none">rovides comprehensive psychological and behavioral case management & therapy services within a continuum of high to low intensity services designed to improve or maintain each mental health consumer's abilities to function in the community. Assesses, monitors, and follows up with consumer's ability to meet basic needs in moving toward recovery. Responsible for all clinical documentation of consumer interaction. For a full description of benefit offerings, please visit: https://wellpower.org/workplace-of-choice/.</p> <p> </p> <p> </p> <p><strong>ESSENTIAL FUNCTIONS</strong></p> <ul> <li>Manage and triage incoming phone calls regarding individuals 17 and younger as well as their</li> <li>caregivers to the Access line.</li> <li>Collaborate with family, professionals and other external providers regarding hospital discharge</li> <li>intakes.</li> <li>Conduct face to face behavioral health assessments and intakes in the office and/or virtually for people 18 and over. In-person availability is required for intakes as needed.</li> <li>Maintain accurate and timely clinical documentation consistent with agency standards.</li> <li>Participate in team meetings, trainings and supervisory sessions as required.</li> <li>Maintain a trauma informed environment of wellbeing.</li> <li>Maintain flexible schedule.</li> <li>Other duties as assigned.</li> <li>Knowledge or ability to learn and practice trauma informed principles and practices.</li> <li>Ability to join with and implement a strengths-based approach to children, families and adults.</li> <li>Knowledge of community and available resources. · Familiarity with crisis intervention techniques and psychotherapy. · Fluent in both English and Spanish (preferred). · Maintain flexible schedule. · Self-directed individual, and also team player. · Knowledge and understanding of trauma-informed care principles/practices. Proficiency with Microsoft SharePoint, Word, Excel, and Teams. · Performs other duties as assigned.</li> </ul> <p> </p> <p> </p> <p><strong>REQUIRED QUALIFICATIONS</strong></p> <p>Masters Degree in psychology, social work, or other related human service degree. Licensure required (LPC, LCSW, LMFT, or Licensed Psychologist).</p> <p> </p> <p> </p> <p><strong>TYPICAL PHYSICAL AND MENTAL DEMANDS </strong></p> <p>Requires sitting, standing, bending and reaching.
