SCHEDULE: Full-time

The 225-bed facility recently completed an $80-million expansion that includes a new Emergency Department, Intensive Care Unit, Outpatient Services Unit, Cardiac Catheterization Laboratory, and The Grossman Burn Center. Position Summary: Organizes and coordinates services and resources to meet patient's healthcare needs; ensures appropriate use of the health care system; provides and manages episodes of care for patients that produce quality outcomes for the same of less cost. Utilizes a process that will provide quality care along a continuum, decrease fragmentation of care across many settings, enhance patient's quality of life and contain costs. Accomplishes these objectives through review and coordination of patient care during the pre-entry, entry, within the organization, pre-exit, and exit phases. Reports To: Director Case Management Supervises: None Case Manager FT/Days Rotating Weekends Case Management

Qualifications

Position Requirements: Licensure/ Certification/Registration:
  • Current Licensure as a Registered Nurse in the state of California required.
Education:
  • Graduate of an accredited School of Nursing Baccalaureate in Nursing or equivalent preferred.
Experience:
  • Utilization management/discharge planning and acute care nursing experience preferred
  • Experience with Interqual and Midas is preferred.
Special Qualifications:
  • Knowledge of discharge planning needs and community resources, governmental regulation for Medicare, Medi-Cal, and Champus patients,
  • Prospective payment systems and third party payer contracts and agreements, and the quality improvement process. Hospital Fire Life Safety required within 60 days of hire.
