JOB PURPOSE OR MISSION: Facilitates and coordinates admissions to the Hyperbaric and Wound Care Units. Determines patients' eligibility for admission through a complete assessment of key data, physical assessment of the patient, and compliance to unit admit criteria. Develops relationships and maintains communications with referral sources, patients, and family members to ensure the delivery of quality patient care. Markets General Health System products and services in a manner consistent with the GHS sales plan. Performs job for the age population served, as defined in the department's scope of service.


PERFORMANCE CRITERIA

JOB FUNCTIONS

ESSENTIAL JOB FUNCTIONS include, but are not limited to:

1. Coordinates admissions to inpatient Hyperbaric and Wound Care Units.

PERFORMANCE STANDARDS:
• Evaluates and assesses referral patients to determine if patient meets admission criteria.
• Conducts phone interview if geographically inaccessible.
• Develops pre-admission plan of care.
• Contacts payor source prior to admission to obtain admission approval to assigned units.
• Contacts family members/significant others prior to admission to the unit, to review admission day process.
• Provides orientation for patients/family members/significant others to assigned units.
• Provides follow up and maintains contact with referrals that do not yet meet admission criteria.
• Maintains accurate and precise log of patient referrals and referral outcomes.
• Completes daily/monthly unit statistics in regards to financial goals of units.
• Demonstrates knowledge of federal (Medicare), State and commercial payor regulations and guidelines regarding reimbursement of services.

2. Serves as resource to other members of the healthcare team, physicians, referral sources and the community.

PERFORMANCE STANDARDS:
• Demonstrates the ability to educate staff, patients and physicians about relevant services at BRGMC.
• Assists in planning and implementation of BRGMC Hyperbaric and Wound Care Units' marketing strategies.
• Acts as community resource and advocate regarding General Health Hyperbaric and Wound Care Units.
• Maintains liaison relationships with contacts by providing information and education on relevant services, payer issues and related areas.

 

3. Builds relationships with community professionals and initiates strategies to generate referrals to GHS programs.

PERFORMANCE STANDARDS:
• Accurately updates GHS customer database with current client information .
• Contributes to GHS sales efforts through customer contacts, within agreed-upon timeframes.
• Promotes specialty clinics in community to highlight GHS programs and services by identifying education needs of customers and organizing and implementing seminars as necessary.

4. Assists the supervisor in the planning and implementation of sales strategies and in identifying programs needed to better serve the community.

PERFORMANCE STANDARDS:
• Maintains accurate monthly reports for areas of responsibility.
• Regularly measures effectiveness of existing strategies and provides feedback to supervisor.
• Maintains daily communication with coworkers to ensure quality service to all customers.
• Maintains and upgrades knowledge of GHS products and services through defined sales meetings and training activities.
• Contributes to system planning efforts through mandatory participation in GHS sales meetings.


5. Performs other duties and special projects as required.

 

 

EXPERIENCE REQUIREMENTS
Two years clinical experience in appropriate modality preferred

EDUCATIONAL REQUIREMENTS
Bachelor's degree preferred.

SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Louisiana State licensure required as LPN, RN, GSW/LCSW, PT, PTA, OT, COTA or ST.

Excellent communication and organizational skills. Ability to work alone and utilize effective time management skills. Proficient in spreadsheet, word processing and data processing.

HIPAA REQUIREMENTS:
Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to Job Position including but not limited to: Medical records without limitation both paper and electronic, patient demographics, lab and radiology results, patient information related to surgery or appointment schedules, medical records related to quality/data, patient financial information, patient billing 3rd party, patients related complaints, information related to patient location, and/or patients' religious beliefs.

SAFETY REQUIREMENTS:
Maintains knowledge of and adherence to all applicable safety practices appropriate to Job Position including but not limited to: Incident reporting, handling of wastes, sharps and linen, PPE, exposure control plans, hand washing, environment of care, patient identification, administers medications/blood products, receives orders for patients, validates patients for any procedures/surgery, transports, monitors or observes patients for procedures including infusion pumps, monitors clinical alarms and collects blood samples.
