RN, Population Health Nurse (Nurse Coach) –
The Population Health Nurse, also known as the “Nurse Coach,” plays a pivotal role in managing a broad patient population and driving performance across key quality metrics aligned with value-based agreements for ProHealth Care and ProHealth Solutions (PHC, ACO).
This role is essential to achieving top-tier Star ratings and advancing system-wide quality outcomes. The Nurse Coach must bring a blend of clinical expertise and administrative skill, with strong proficiency in data managementnincluding advanced Excel and pivot table functionality.
Key responsibilities include:
Overseeing population-level care strategies and performance metrics
Collaborating with providers and interdisciplinary care teams
Engaging patients through targeted beneficiary outreach programs
Navigating and analyzing complex data sets to inform care decisions
Adapting quickly to shifting priorities to sustain excellence in quality performance

Schedule & Location:
Full time (0.9 FTE, 36 hours/week)
Day shift, Monday–Friday, 8:00 am – 4:30 pm

Collaborates with physicians, advanced practice providers, and ancillary health care team members to coordinate care. Operates from an in-depth understanding of the importance of total health management and supports the patient and family/caregiver choices. Manages a large population on our value based agreements through our ACO. Activities include - pre-visit readiness; coordination of care; gaps in care completion and performance improvement activities that continue to develop the team based health care concept.
What you will need:
- Bachelor's Degree in Nursing required (enrollment in BSN completion program will be considered). Master's Degree in Nursing preferred.
- Current State of WI or eNLC State RN License.
- BLS (CPR) Certification.
- Certification in Guided Care Nursing, Nursing Case Management and/or Gerontological Nursing preferred.
- At least 5 years of hospital-based nursing experience, adult primary care clinic, or home care experience.
- RN case management, team lead and/or other nursing leadership experience preferred.
- Basic understanding of computer use: Microsoft Office programs, internet, and health information technology (EHR).
- Experience with Epic EMR preferred.
