Description

Provides education and coaching to rising and moderate risk members with chronic conditions. Completes assessments and develops individualized care plans for members with the goal of improving self-efficacy in managing their chronic condition(s). Collaborates with both internal and external care team members. Assists manager with staff training, assigning and monitoring work queues and standard operating procedures.

Responsibilities

  • Contacts member case assignments with chronic health conditions by phone to discuss health conditions.
  • Assesses chronic health condition self-management needs and creates individualized care plan to assist member. Delivers interventions from the care plan to meet the member's goal on an ongoing basis.
  • Provides resource connections and interventions to close care gaps.
  • Communicates and collaborates with internal and external contacts and facilitates care with other providers and community health care resources.
  • Orients, trains, assists and reviews work of more junior level staff.  Assists management with case assignments, monitoring work queues, and recommending, creating and implementing workflows.
  • Performs other duties as assigned.


Qualifications

Qualifications

Member Health & Wellness Coach

  • Bachelor's degree in health science, public health, counseling, or related field. Master's degree a plus.
  • 2 to 3 years of experience working with individuals to improve their health and demonstrate healthy lifestyle skills, preferably in a health plan, and/or clinical or community health setting.
  • Motivational Interviewing experience required.
  • Motivational interviewing training/certification required
  • Nationally Board-Certified Health and Wellness Coach (NBC-HWC)
  • Certified Health Education Specialist (CHES)
  • Master Certified Health Education Specialist (MCHES)
  • Licensed Practical Nurse (LPN) a plus.
  • Knowledge of chronic conditions, managed care processes, and efficient care delivery processes.
  • Knowledge of Motivational Interviewing of Transtheoretical Model behavior change techniques.
  • Knowledge of chronic condition self-management coaching techniques.
  • Understanding of effective lifestyle behavior change such as tobacco cessation and weight management, and the role lifestyle plays in conditions such as diabetes and heart disease
  • Strong interpersonal relationship & communication skills, and the ability to work autonomously with a diverse patient population.
  • Intermediate Microsoft Office skills and proficiency navigating Windows-based programs and applications and web-based systems.
  • Ability to work independently and meet deadlines.

Senior Member Health & Wellness Coach

  • Bachelor's degree in health science, public health, counseling, or related field. Master's degree a plus.
  • 3 to 5 years of experience as a Member Health & Wellness Coach or equivalent experience working with individuals to improve their health and demonstrate healthy lifestyle skills, preferably in a health plan, and/or clinical or community health setting.
  • Motivational Interviewing experience required.
  • Motivational interviewing training/certification required.
  • Nationally Board-Certified Health and Wellness Coach (NBC-HWC)
